Book an ergonomic assessment
Book an assessment if any of the following applies:
Stage two symptoms
My symptoms started more than 12 weeks ago, and I am having difficulty with some of my daily activities.
Stage three symptoms
My pain/discomfort is constant, it disrupts my sleep, and my daily activities are affected.
Referral
I received a referral from the Ergonomic Program Advisor to book an assessment.
Choose your area of work
Ergonomic assessments must be completed in a timely manner to help prevent further injury.
The external service provider, EWI Works, completes in-person ergonomic assessments. For special circumstances a virtual assessment will be completed. Applicable fees apply - department approval is required.
- Complete the EWI ergonomic assessment approval form and submit your approval form within 3 business days of your assessment referral recommendation.
- Once EWI receives your approval form, they will contact you within two business days to schedule your appointment and provide pricing information.
- Once the assessment is completed, the report recommendations will be available within five business days.
Departments are responsible for approval and purchase of assessment fees and furniture /equipment recommendations, related to your permanent workstation. Refer to your Blue Cross Wellness Account for equipment and furniture not related to your permanent workstation.
Multiple assessments booked consecutively receive a discounted rate. Please contact Donald MacDonald at EWI Works for more information (phone: 403-802-0886).

- If you have missed time from work due to your illness or injury, or your injury is work related, follow the instructions on the Illness and Injury website.
- Notify your leader about your concerns and advise them you are seeking ergonomic assistance.
- Complete the Ergonomic Consultation Questionnaire.
- Notify the Ergonomic Program Advisor that you require an ergonomic assessment.
If your concern is related to a computer workstation, follow the information above for office workers.
