Please ensure you have carefully reviewed all application steps including the Sample Application prior to submitting your application. Each applicant is only allowed one submission. Applications that do not adhere to our guidelines will be ineligible.

Instructions on how to convert a Word Document to PDF:

  1. Open your completed Word Document application
  2. Click File > Save as Adobe PDF
  3. Appropriately name the file using the template, “Faculty_FirstName_LastName_Application”

How to organize, add, or delete pages of a PDF:

  1. Open PDF file
  2. Click Organize Pages on the right hand panel
  3. Click and drag pages to desired page order
  4. Choose Insert > From File to add an existing page or pages
  5. Choose Delete to remove any undesired pages
  6. Choose Extract to remove and save an existing page or pages
  7. Click File > Save As
