How to Register with the Disability Resource Centre

If you have been diagnosed with a disability and have a copy of your documentation:
  1. Please bring in (or fax) your documentation. For:
  2. Please bring in (or fax) a copy of our Registration and Consent form.
  3. Once we receive this form, and your documentation, we will book you in with a DRC advisor. Your advisor will review your accommodations and discuss possible supports.
If you have been diagnosed with a disability but do not have a copy of your documentation:
  1. Please bring in (or fax) a copy of our Registration and Consent form.
  2. Please sign our Release of Information form. We will then request a copy of your documentation on your behalf.
  3. Once we have received your documentation, we will contact you to book an appointment with a DRC advisor. Your advisor will review your accommodations and discuss possible supports.
If you have never been diagnosed with a disability:
  1. Please fill out and bring in (or fax) copies of the Delta Screener form, and Screening Checklist for ADHD form.
  2. In addition, we will need you to fill out our Registration and Consent form.
  3. Once we have received all of the above forms, we will book you an appointment with a DRC advisor to discuss the next steps.
A few things to note...
  1. Our services are of no cost to you.
  2. Our services are private and confidential, and therefore will not show up on your transcripts.
