How to Register with the Disability Resource Centre
Note: These forms require Acrobat Reader to be opened. This program may be downloaded here for free.
If you have been diagnosed with a disability and have a copy of your documentation:
- Please bring in (or fax) your documentation. For:
- A diagnosis of ADHD: we require a copy of your psychoeducational assessment, or please have your physician, psychologist or psychiatrist fill out the Verification of Psychological or Psychiatric Disability form
- A diagnosis of a Learning Disability: we require a copy of your psychoeducational assessment, or please have the assessor (psychologist) or psychiatrist fill out the Verification of Psychological or Psychiatric Disability form
- A medical disability: we require a letter from your physician or the Documentation of Medical Condition form filled out
- A mental health disorder: we require a letter from your psychologist or psychiatrist, or you can have them fill out the Verification of Psychological or Psychiatric Disability form
- Please bring in (or fax) a copy of our Registration and Consent form.
- Once we receive this form, and your documentation, we will book you in with a DRC advisor. Your advisor will review your accommodations and discuss possible supports.
If you have been diagnosed with a disability but do not have a copy of your documentation:
- Please bring in (or fax) a copy of our Registration and Consent form.
- Please sign our Release of Information form. We will then request a copy of your documentation on your behalf.
- Once we have received your documentation, we will contact you to book an appointment with a DRC advisor. Your advisor will review your accommodations and discuss possible supports.
If you have never been diagnosed with a disability:
- Please fill out and bring in (or fax) copies of the Delta Screener form, and Screening Checklist for ADHD form.
- In addition, we will need you to fill out our Registration and Consent form.
- Once we have received all of the above forms, we will book you an appointment with a DRC advisor to discuss the next steps.
A few things to note...
- Our services are of no cost to you.
- Our services are private and confidential, and therefore will not show up on your transcripts.
